Available Screenings & Tests

WellLife offers DOT physicals completed by certified medical examiners. WellLife aims to partner with drivers and employers to create an efficient and smooth process to obtaining your medical certificate. For your best experience, please read the Full Preparation Instructions and complete the necessary paperwork to bring to your physical exam. Please arrive at least 30 minutes early to complete paperwork.

Obtaining safe medical clearance to operate a commercial motor vehicle is the goal of the driver, the employer, and the medical team.

Bringing the required documentation to your medical exam can make this appointment smooth and efficient.

WellLife can provide pre-employment physical exams as well as pre-employment urine drug screens. Patients should bring their health history information and their job description to the appointment. WellLife providers will review these with the patient and confirm if they are capable of performing the expected job functions.

WellLife can administer urine drug screens and breath alcohol testing for pre-employment, random screenings, reasonable suspicion, and post-accident. The patient should present with an employer preferred chain of custody form or an Escreen passport. A valid photo ID with name and date of birth must be provided by the patient. For all breath alcohol tests, employer DER (designated employer representative) must be provided at time of testing.

OSHA mandates that employees that work with hazardous airborne particles are required to use respirators to protect themselves. For some, an OHSA medical questionnaire will be sufficient while others may require full medical clearance. WellLife can provide these services and exams to determine if the employee can safely tolerate the respirator. We also provide onsite spirometry testing (lung function testing).

WellLife can provide a baseline audiogram assessment for purposes of maintaining a hearing conservation program.

On-Site Employer Services

WellLife's onsite services are available for any employer that desires health & wellness services for their employees. Employers do not have to be a contracted WellLife client and can be charged on a per service basis. Services are customizable and include the following:

  • Biometrics
  • Group classes
  • Vaccines
  • Physical exams
  • Blood pressure checks
